Unfortunately, the important factor in what makes surveys really work is not in the technical construction of the survey itself, but rather it's the questions and how they are written.

Here's a survey tool I'd like to see. One in which the user can specify:

this survey is to appease me and my ego -- build a survey of 5 questions designed to tell me what I want to hear.

This survey is to get real feedback, regardless -- For real, tell me what do you think about this thing?

That'd be doing something impressive.

One interesting and useful feature is the ability to fully customize the surveys through the use of CSS. From a designers perspective, it is important to be able to brand everything sent out by a company. Too often you see generic, ugly surveys.
